2. Trend signal — why now?

This is a regulatory-arbitrage play with a fresh, dated trigger.

In March 2026, IRDAI tightened the screws: insurers must settle 100% of genuine claims within 15 calendar days of the final discharge summary, with compound-interest penalties paid from shareholder funds if missed — and the May 29, 2024 Master Circular’s hard clocks remain in force: 1 hour for initial cashless authorization, 3 hours for final discharge authorization once the hospital sends the final bill and discharge summary. The penalty money explicitly cannot be deducted from the patient’s sum insured.

At the same time, the General Insurance Council’s Cashless Everywhere initiative (2024, expanding through 2025–26) pushed cashless volume into hospitals that were never empanelled — exactly the 25-bed nursing homes that have no TPA infrastructure. And the economic backdrop: ₹30,000 crore of health-insurance claims were denied in FY25, and IRDAI is publicly “cracking down.”

The pain is vivid and on the record: “You are sitting on the edge of the hospital bed. Your bags are packed. The doctor cleared you at 10 AM. It is now 4 PM. The non-obvious wedge: there’s now a legally binding clock with a cash penalty attached, but the small hospital — the party that gets blamed by the angry family — has no instrument to prove who is late. When the clock breaches, the hospital eats the room-day or the patient pays cash and fights a reimbursement. Nobody is timing the TPA.

3. The opportunity

The incumbents are heavy Hospital Information Systems with a TPA module bolted on — Attune ClaimBook, CureNearMe, MocDoc, ACGIL, Dataman. They are sold as full HMS implementations: multi-month rollout, IT-budget pricing, designed for 100+ bed hospitals with a dedicated insurance team. ClaimBook’s pitch is auto-parsing and HIS integration — i.e. it assumes you already run their HIS.

What none of them do: stand alone, install in a day, and operate the IRDAI clock as an offensive instrument. The 40,000 sub-30-bed nursing homes don’t run an HMS — they run a TPA desk on the insurer’s web portal, email, and a WhatsApp group. They have no system that says: “Pre-auth request sent 11:04. IRDAI 1-hour clock expires 12:04. No response. Auto-escalating to insurer grievance officer + TPA nodal with the regulation citation and the timestamped proof.”

The 10× isn’t better claim filing — plenty of tools do filing. It’s converting a vague “the TPA is slow” complaint into a timestamped, regulation-cited, auto-escalated breach record that (a) gets the patient discharged faster, (b) moves blame off the hospital and onto the insurer, and (c) builds the evidence file for the compound-interest penalty IRDAI now mandates. That’s a workflow no HMS sells because HMS vendors sell to the buyer who has staff to absorb the delay.
